Seeking a dedicated Family Nurse Practitioner (FNP) or Physician Assistant to provide full-scope primary care in a relationship-based model.

  • Provide primary care services including well-child visits, school physicals, immunizations, and chronic disease management
  • Conduct health education and promotion for individuals and groups
  • Administer or prescribe treatments and medications as appropriate
  • Order, perform, and interpret diagnostic tests and lab work
  • Evaluate and treat minor illnesses and injuries
  • Maintain accurate and timely documentation in medical records
  • Refer patients for specialty services when needed
  • Perform procedures within scope of competency
  • Uphold strict patient confidentiality and clinic protocols
  • Collaborate with medical leadership on additional services as needed
  • Serve as a key provider resource for patients navigating chronic conditions, preventive care, and community health services.
  • FQHC environment


Qualifications:

  • Completion of MSN and accredited Family Nurse Practitioner program or accredited Physician Assistant program
  • Current Oregon license to practice medicine, DEA license, and CPR certification
  • Current BLS certificate
  • Strong communication and teamwork skills
  • Ability to work independently and collaboratively in a fast-paced setting
  • Commitment to confidentiality and culturally responsive care
  • Willingness to work occasional evenings and weekends
  • Experience in FQHC or underserved settings preferred
