Position Highlights:
- Full-time opportunity supporting a Same Day Access Clinic
- Provide same-day clinical care for patients
- Focus on transitional care visits, including hospital discharge follow-ups
- Evaluate, diagnose and treat acute and subacute conditions
- Coordinate care with primary care providers and care teams
- Patients scheduled continuously throughout the day
- 8–9 hour shifts
- Flexible scheduling options may be available
- Work in a fast-paced, collaborative, team-based environment
- Help improve patient access and support positive care outcomes
Qualifications:
- Current Massachusetts licensure as a Nurse Practitioner or Physician Assistant
- Board certification required
- Experience in primary care, urgent care or transitional care preferred
- Strong clinical decision-making and communication skills
- Ability to work efficiently in a dynamic same-day access setting
