Highlights:
- Full-Time, Monday through Friday, 8:00 AM - 5:00 PM
- The Medical Director will see patients in their homes up to 3 days a week alongside their provider colleagues
- The Medical Director will be responsible for leading, as a dyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.
- Additionally, we see this leader being a key thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program
- Geriatric certification welcome but not required
Core Responsibilities:
- The Medical Director's primary duties are focused on the management of their teams.
- Key responsibilities include:
- Leading At Home providers and ensuring their priorities and outcomes are in line with those of the organization
- Providing timely and constructive feedback to providers so the medical group continues to improve performance for patients and colleagues
- Helping providers master the data-driven tools and practices required to keep patients well and out of the hospital, when avoidable
- Reviewing regular quality dashboards with providers, coaching on tools to improve quality outcomes
- Coach providers on appropriate coding and documentation
- Provide managed care experience/teaching to providers
- Additionally, the Medical Director will be part of the At Home leadership team that will have strategic input into a variety of divisional projects that support the enhancement of the care model, including:
- Medical management, care management, utilization management, and quality improvement tools and processes
- Development of clinical compliance policies and guidelines
- Representing external partners (e.g., vendors, payers, providers) and creating relationships and partnerships that support our mission and our value-based model
- Additional responsibilities include:
- Understanding and communicating provider performance/compensation plans and being able to answer questions and deliver results to direct reports
- Serving as formal manager doing regular performance reviews and check-ins, approving paid-time-off requests, and managing miscellaneous human resource topics for providers in the market
- Drive Culture: we foster a culture of innovation and open dialogue, and we work with urgency. This requires passion and enthusiasm, active listening skills, and comfort with and commitment to regular change. Our Medical Directors are expected to support this culture and set an example for others to follow.
