A community-focused healthcare organization in Fremont, California, is seeking an experienced, Spanish-speaking Licensed Clinical Social Worker (LCSW) to provide integrated behavioral health services and clinical consultation within a primary care setting.

Position Highlights:

  • Provide integrated behavioral health services within a primary care setting
  • Consult with primary care providers regarding mental health, substance use, and psychosocial concerns
  • Support patients with behavioral health needs related to chronic pain and chronic disease
  • Conduct behavioral health screening, triage, evaluation, and assessment
  • Provide individual, family, and group interventions, including CBT and contingency management
  • Provide crisis intervention and behavioral health support
  • Offer group treatment, psychoeducation, and health education
  • Collaborate with a psychiatrist regarding psychopharmacological consultation within scope of practice
  • Work closely with medical case managers and interdisciplinary healthcare providers
  • Provide education and support to patients and families
  • Coordinate referrals to appropriate community resources
  • Provide comprehensive services including intake, treatment planning, counseling, collateral intervention, and outreach
  • Participate in clinical case conferences, peer review, quality assurance, and program evaluation
  • Provide behavioral health consultation and training to clinic staff
  • Maintain accurate clinical documentation, case records, reports, and program materials
  • Contribute to ongoing development of behavioral health procedures and protocols
  • Practice in a multicultural, multilingual community health environment


Qualifications:

  • Master’s degree in Social Work/Social Welfare or doctoral degree in Clinical or Counseling Psychology
  • Current California LCSW or Psychologist license
  • Current LPCC license
  • Fluent Spanish-speaking ability highly desired
  • One year of directly related experience preferred
  • Experience in primary care consultation, behavioral medicine, health psychology, or related settings preferred
  • Community health experience and/or experience serving low-income multicultural populations preferred
  • Strong clinical judgment, problem-solving, communication, and collaboration skills
  • Commitment to culturally and linguistically sensitive care
  • Ability to work independently and effectively as part of a multidisciplinary team
  • Willingness to work a flexible schedule as needed

Compensation & Benefits:

  • $92,200–$106,878.24 annual salary
