Position Highlights, Schedule & Responsibilities
- Help design and grow a high-risk primary care clinic addressing patients' physical, mental and social needs.
- Conduct in-depth assessments and develop individualized care plans.
- Collaborate with behavioral-health specialists, care coordinators and nursing, supported by strong administrative resources.
- Address barriers such as housing, employment and transportation.
- No set daily patient quota or volume-based pressure; prioritize engagement and comprehensive care.
- Significant clinical autonomy and opportunities to shape workflows and patient-centered protocols.
- Full-time, 40-hour position with variable weekday hours; the posting also lists rotating weekends and holidays.
Compensation & Benefits
- $125.6976-$180.8481 per hour.
