Seeking a Spanish-speaking BC/BE Family, Internal or Geriatric Medicine Physician to join a collaborative PACE team in the Boston, MA area, providing compassionate, patient-centered care that helps older adults remain safely and independently in their homes while enjoying an exceptional work-life balance.
Position Highlights:
- Full-Time position
- Monday–Friday, 8:00 AM–4:00 PM schedule
- No nights, weekends, or holidays
- Provide care in the clinic, participants' homes, and skilled nursing facilities
- Perform comprehensive physical examinations and routine follow-up care
- Evaluate and treat acute illnesses and post-hospitalization needs
- Utilize telehealth when clinically appropriate
- Participate in hospital rounding and a shared on-call rotation
- Collaborate with an interdisciplinary team to develop individualized care plans
- Manage chronic conditions, preventive care, and palliative/end-of-life care
