Seeking an experienced Nurse Practitioner to join our dedicated team delivering high-quality care to older adults in the greater Rutland, Vermont area.
Details:
- Responsibilities include conducting annual wellness visits, along with other routine and specialized assessments, to ensure comprehensive care for older adults
- 3-5 years Primary Care Practice Experience or geriatric setting preferred
Essential functions include though are not limited to
:
- Obtains complete medical history and physical examination on patients.
- Reviews medical records
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures
- Establishes presumptive diagnosis
- Orders appropriate laboratory and diagnostic procedures
- Develops a therapeutic plan for patient
- Sutures wounds
- Prepares written prescription orders for drugs and controlled substances
- Instructs patients and family regarding medications and treatment instructions
- Provides recommendations regarding health and illness prevention
- Recommend community resource to meet patient/family needs
- Performs independent evaluations and treatment procedures in emergency situations
- Provides monitoring and continuity of care between physician visits
- Maintains patient confidentiality
