A community-focused healthcare organization in San Jose, California, is seeking an experienced Advanced Practice Provider (APP) Lead to provide primary care while supporting the onboarding, development, and clinical success of fellow Nurse Practitioners and Physician Assistants.

Position Highlights:

  • Provide primary care to patients of all ages within scope of practice
  • Serve as a key resource for NP and PA onboarding, training, and mentorship
  • Support APPs with clinical workflows, documentation, productivity, and clinic protocols
  • Provide ongoing coaching to promote clinical excellence and professional development
  • Serve as a clinical role model and resource for APP staff and the broader care team
  • Lead and participate in quality improvement initiatives, including medical record audits and clinical quality metrics
  • Assist Clinical and Medical Directors with provider-related issues and operational needs
  • Support training and skill development to improve provider productivity and efficiency
  • Help develop, review, and update policies, procedures, protocols, and standardized procedures
  • Provide comprehensive examinations, medical workups, testing, diagnosis, treatment, prescribing, and follow-up care
  • Perform procedures including injections, immunizations, medication administration, and wound suturing
  • Consult with physicians when patient needs fall outside the APP’s scope of practice
  • Collaborate with RNs, LVNs, MAs, Clinic Managers, Supervisors, and other healthcare leaders
  • Maintain a personal patient panel while meeting productivity and documentation standards
  • Provide clinical consultation and triage support to clinic staff during operating hours
  • Participate in strategic initiatives, grants, community events, health fairs, presentations, and special projects
  • Collaborate with Compliance and leadership teams to address complaints, grievances, and organizational priorities


Qualifications:

  • Minimum 1 year of recent experience in a busy outpatient primary care setting serving multigenerational, multicultural patients
  • Nonprofit community clinic experience preferred
  • Spanish or another second language desirable
  • Current, unrestricted California Nurse Practitioner or Physician Assistant license
  • Current national certification through the appropriate certifying organization
  • Current CPR certification
  • Valid DEA license
  • NPI number

Compensation & Benefits:

  • $168,150–$194,919 annual salary
  • Additional benefits available
