The APRN – Primary Care
who is either licensed as Family practice FNP or Adult-Gerontology AGNP working with an adult population of age 18 and up in an Rural Health Center (RHC), collaboratively with physicians, providers, and clinical staff to deliver high-quality, patient-centered care to an assigned patient panel. This role includes comprehensive assessment, diagnosis, treatment planning, and ongoing management of adult patient over their lifespan within the scope of APRN licensure and medical staff privileges.
The APRN plays a vital role in promoting preventive care, managing chronic conditions, educating patients and families, and contributing to a positive, team-based clinic environment.

Located on the edge of the White Mountain National Forest, the area sits along the Connecticut River at the mouth of the Ammonoosuc River. You're surrounded by beautiful terrain and mountains, state and national parks, wildlife habitat, and public recreation. The area is known for its covered bridges, beautiful fall foliage and Lupine Festival. Named as a top 10 Rural County to live in, the Appalachian trail runs through at least 10 towns in Grafton County. This is also ski country, with several down hill ski centers in the area, as well as opportunities for cross country skiing and snow tubing. No sales tax!
